• Accomplished Agreement (Kasunduan) Form
• 2×2 Picture
• Bureau of Fire Protection Clearance
• Electrical Sketch / Layout / Plan
• Community Tax Certificate (Sedula)
• Pre Membership Seminar (District Offices every Wednesday at 9:00 Am)
• Barangay Clearance
• Photocopy of Valid ID

Transfer / Change of Account Name

• 2×2 Picture
• Community Tax Certificate (Sedula)
• Pre Membership Seminar (District Offices every Wednesday at 9:00 Am)
• Barangay Clearance
• Letter authorizing the transfer rights of the kwhm duly signed by the old member
• Photocopy of a valid ID of the old member
• Photocopy of valid ID of the New member
• Kasunduan form and Change name form duly filled up and signed
• In case of deceased member, the new applicant shall submit a photocopy of death certificate of the old member
• In case of acquisition of property, the new applicant shall submit a photocopy of the deed of sale/donation
